I can't figure out how to combine both counties and cities in a manually selected region. Cities do not even show up as an option. I need to build different combinations of political jurisdictions (all either cities or counties).

'City' is not a well defined term. You'll probably notice that where the
USA population was once 95% rural, it is now more like 5% rural
and population concentrations are making super-cities that cross
county lines and state lines.
You'll need to use MSA's (Metropolitan Areas in the Territories Wizard).
Anything further will require importing shapefiles that will be unmanaged.
I think the US Census site will have links for these political shapefiles.
