It is a pain. :x I'd really like to be able to control the saving of .PTM files so that routes created in our application can be saved and downloaded to PocketStreets on a PDA for the Bus driver. 8)

You can use the .SaveMap and .SaveMapAs methods of the MapPoint Control, both save a .PTM file to a specified path with the contents of the ActiveMap of the MapPoint Control in use.
